- VUmc patient identification card
- Identification (passport, drivers license)
- Complete list of medications (alternatively, bring the medicine boxes with you)
Children < 16 years old should be accompanied by an adult.
We advice you to wear comfortable clothing.
It is not necessary to be sober for your preoperative screening visit.
