Questionnaires (PRO)
Dear test administrator,
Please find below the questionnaires (in Dutch, English, French, German and Italian) you need in order to participate in the 1419 trial.
IMPORTANT: the French and German HADS questionnaire is not yet included in the documents below and will be provided later. Please administer the other PRO questionnaires as instructed. The French en German HADS questionnaire should be sent to the patient when it is available.
To open the documents you will need to use a password. If you don't already have one, please contact EORTCsupport@amsterdamumc.nl.
